Can my employees pay for services?

Employees can pay for certain pre-hire and annual services before their visit with a credit or debit card. We can make a note on your account that services should be paid by the employee. 

Our Employer Solutions team can advise you on what services a patient can pay for and/or help you update your account notes. Contact your Business Development Consultant at [email protected] or call 260-373-9017. 

What is the Agility Employer Portal?

The Agility Employer Portal is a secure and easy-to-use tool that streamlines communication between your leaders and our clinical team. 

  • Get 24/7 access to drug and alcohol testing results. 
  • Access past and current employee work status reports. 
  • View past visits for employees and review the services provided at each visit. 
  • View OSHA and other safety information related to employee incidents. 
  • Get compliance, safety and case management reports on demand. 
  • View all results and reports in a downloadable and printer-friendly format.

How do I sign up for the Agility Employer Portal?

It is recommended that one company representative be designated as the portal administrator. The administrator will have access to and be responsible for adding new users, removing old users, unlocking accounts, and resetting passwords.

Authorizations, Forms & Treatment Process

What is the process for treatment?

Our staff will provide authorized services and treat emergent injuries. If no authorization is provided, our staff will treat the patient by following the company account previously established with your Business Development Consultant (unless there is an account note indicating that authorization is required). 

If no services are listed on your account, we will attempt to contact your company to obtain authorization for services to be performed. (This may cause delays for the employee.) If we are unsure what services a patient needs, no authorization is present and we are unable to reach your company representative, the patient will be referred back to you, their employer.

Can I send my company forms with my employee to be completed by the provider during their visit?

Yes, companies can provide their own forms for completion during the visit. There is an additional fee for completing external forms. We must be made aware of this service so we can add a reminder to your account. Some external forms can be saved in your account. Our medical director makes all final decisions on whether providers can sign external paperwork.

How do I authorize my employee for services/treatment?

A Parkview custom authorization must be submitted. To request your custom authorization form, contact our Employer Solutions team at [email protected] or call 260-373-9017. 

Other authorization options: company- or TPA-designed authorization form, verbal authorization by a company contact at the appointment or verbal authorization by calling the Occupational Health Call Center at 260-373-9300, option 1. 

Where do I submit my completed and signed custom authorization form?
  • Send to the appointment with your employee (preferred method to avoid delays). 
  • Fax to 260-458-5836. 
What COVID services and resources does Occupational Health provide?
Parkview Occupational Health is no longer offering pre-hire or exposure COVID-19 testing. 

Scheduling & Appointment Management

How can appointments be scheduled?

Appointments can be made by emailing [email protected] or calling the Occupational Health Call Center at 260-373-9300, option 1. We recommend employees have their custom authorization in hand so they can refer to it and answer questions.

If you are an employer attempting to schedule an appointment for an employee, you can complete the Appointment or Results Request Form and email it to [email protected]. Once the form is submitted, our team will contact you to confirm the appointment date, time and location. This form is NOT an authorization for services. Please send a Parkview custom authorization with your employee to their appointment.

How can I find out if an employee has an upcoming appointment, has had an appointment, what services were performed and what time they arrived and left the clinic?

Email [email protected] or call the Occupational Health Call Center at 260-373-9300, option 1 for appointment details. 

If you use the Agility Employer Portal, this information can be found under the Visits and Scheduling tab on your dashboard. 

Drug Screens & Compliance

How long does it take drug screen results to process?

Negative rapid drug screen results are available shortly after the appointment. 

Results for negative non-rapid drug screens generally take 48 hours to process. Results for positive non-rapid and rapid drug screens sent for further testing generally take 5-7 business days to process.

How do I request missing or incomplete results?
If you are inquiring about drug screen results, email [email protected] or call 260-373-8571, 260-373-8572 or 260-373-8573. 

For all other results, contact 260-373-9300, option 1, or email to [email protected].

Who do I contact for questions about DOT drug screen compliance and/or a Consortium program?

For questions about DOT drug screen compliance for the employee or employer, visit the U.S. Department of Transportation website – https://www.transportation.gov/.

Do you have an eScreen National account?
You can now send your employees to their appointment with an ePassport. Clinic staff will scan the barcode and process results and billing via your established eScreen National account. 

Results Delivery & Technical Help

I cannot open the encrypted email I received to access my employee’s results.

Email encryption protects potentially sensitive information from being read by anyone other than the intended recipient. Refer to this resource on how to retrieve an email encrypted by Office 365.

I am not receiving emails with results

You may need to request that your email team add emails from @parkview.com and [email protected] to your whitelist (safe email senders).

I’m locked out of the Agility Employer Portal.

If you are the portal administrator, email [email protected] or call 260-373-9017, and we will send password reset emails. 

If you are a portal “user,” contact your portal administrator to have your password reset. Our team can only unlock portal administrators. 

To avoid getting locked out, click change password after the second attempt and answer the security questions.

I cannot find results in the portal or have questions about the portal.

I have a question about an injury/work status or another medical-related question.

Billing & Invoices

How do I make changes to my account (contacts, company name, company address, protocols, billing)?
Contact our Employer Solutions team by reaching out to your Business Development Consultant at [email protected] or call 260-373-9017. 
Can I switch billing to a P.O. Box, work compensation carrier or email?

We offer all these options. To bill a work compensation carrier, we need the carrier’s name and address. 

If you would like to view your invoice before forwarding it to your work compensation carrier, we don’t need to know your work compensation carrier information.
