Dedicated Clinic
The dedicated clinic model is an on-site, employer-specific clinic that aims to bring employees greater access to primary care, annual wellness visits and chronic condition management and education. Dedicated Clinic partners include: Fort Wayne Community Schools, Garrett-Keyser-Butler Community Schools, Northwest Allen County Schools, Challenge Tool, and Indiana Tech. Dedicated clinics are best suited for employers that are prepared to invest in clinic operations and ongoing overhead.
Virtual Clinic
A virtual clinic option is also available, allowing employees to connect with a provider from a private on-site space. It supports primary care needs—including wellness visits and chronic condition management—with basic equipment provided. Visits are billed to insurance, with an opportunity for a direct billing agreement.
