The Parkview Advantage Clinic (PAC) offers a team-based approach to address the medical and social needs of patients with two or more chronic conditions who have Medicare Advantage UHC, Humana, Aetna and Anthem insurance plans. At the PAC, your care will be provided by a team of two physicians, a full-time pharmacist, a licensed clinical behavioral therapist and a nurse care navigator.
