The Parkview Advantage Clinic (PAC) is a new option designed to offer a more holistic approach to caring for older adults. Led by Carol Garrean, MD, PPG – Internal Medicine, and Eileen Muzzillo, MD, PPG – Internal Medicine, PAC promises a more comprehensive, team-based approach to navigating the complexities that come with managing multiple chronic health conditions, as well as the changes that accompany aging.
 

The mission

For so many reasons, care can be very confusing for the senior population. Keeping track of and taking multiple medications, scheduling, attending and understanding frequent doctor’s appointments, etc. is overwhelming, particularly if you have physical or resource challenges.

With a multi-specialty, multiteam approach, PAC pulls in the caregivers–the family members– and gets everyone involved, so that the patient and their loved ones, as well as their care team, are on the same page and have the resources and support they need.

Key benefits of PAC

In an effort to provide clarity and support to patients and families, the PAC clinic provides:

Great same-day access – If patients aren’t feeling well, they can typically be seen within the same day.

Longer appointments – Designed to understand the patient’s health history, as well as current circumstances and/or challenges.

Access to a pharmacist – On site to help review medications and offer guidance to patients with multiple prescriptions.

Access to a behavioral therapist – Focused on managing the stress and emotions that accompany physical changes and setbacks.

Access to a care navigator – This team member can help connect the dots for patients and caregivers when it comes to appointments, rescheduling and other resource needs.

Transportation – The PAC team can help coordinate transportation to and from appointments if that’s a barrier.

Personal treatment plans – If a patient has a specific healthcare concern, doctors Garrean and Muzzillo provide tailored, individualized care plans.
 

Who is PAC for?

Senior patients who have been diagnosed with chronic medical conditions such as diabetes, heart disease, heart failure, chronic kidney disease, arthritis, etc.

Ultimately, the goal is to prevent the need to go to the emergency room, or worse, be hospitalized.

The team at PAC thinks about the patient’s goals, as well as the family’s goals, what they want and what’s best for them. When everyone aligns, the result is better health and longer, happier lives for seniors.
