What is covered by insurance?

We encourage you to reach out to your insurance provider to verify your health coverage. All visits are billed to your insurance provider.
 

Is there a startup fee for the program?

There is not a startup fee for our program.
 

What is the cost of the meal replacements and are they covered by insurance?

This will depend on the plan that you choose. They are not covered by insurance. The ESTIMATED weekly cost would be between $35.00 - $58.00 before tax. Parkview co-workers receive a 20% discount.
 

Is a referral required from your primary care provider?

A referral from your primary care provider or specialist is required.
 

How long does the program last?

Our program is open ended. We encourage you to reach your 5% - 10% weight loss goal. You may continue in our program until your weight loss goal is met.  
 

How often would I see the nurse practitioner? Dietitian?

You will meet with the dietitian initially every two weeks and with the nurse pratititioner once a month. These visits may become less frequent depending on your progress.
 

What are the different plan options?

Very Low Carb Meal Plan  |  Low Calorie Meal Plan  |  1200, 1500, 1800, or 2000 Calorie Meal Plans |  All grocery store food plan  |  Medications as determined by the provider
 

What is the cost of the exercise program with the Parkview Health & Fitness Center?

Your cost for a 6-month membership is $60.00. You can also choose to meet with the exercise specialist for a one-time consultation fee of $20.00.
 

Do I have to continue on the same diet as I go through my journey?

The program is individualized for you. Diets may change as you progress through your journey, however consistency is key.
