Opportunities for thoughtful group discussion about end-of-life care are available virtually using Zoom each month. Whether participating individually or with family members, the virtual platform provides a safe way to engage in this important conversation. 

You may have preferences about the care you want to receive if a sudden, serious illness or accident would occur. What if you are unable to speak for yourself at that time? Have you shared your thoughts with your loved ones and your physician? 

Discussion groups are facilitated by Erica Downing, BSW, LBSW, Advance Care Planning, Conversations include these topics:

• Your healthcare values and goals
• Healthcare choices you may have to make in the future
• How to talk about your choices with your doctor and loved ones
• Making a written plan for your advance directives
