Depending on your employer’s benefits and workplace safety programs, you may be eligible for a variety of Occupational Health services that help keep you safe, healthy, and productive on the job. Our team works closely with employers to provide coordinated, convenient care that supports both workplace safety and your overall well-being. In addition, certain services are available directly to the public and can be scheduled without an employer referral.

Some services are available directly to individuals with no employer referral needed. These services can be scheduled independently:

Services available through your employer

Depending on your company’s benefits and safety programs, you may have access to:

  • Work-related injury care
  • Workers’ compensation coordination
  • Drug and alcohol testing
  • Pre-employment and annual physicals
  • DOT physicals
  • Respiratory fit testing
  • Vaccinations and immunizations
  • Health screenings
  • Case management and return-to-work support

These services are provided in collaboration with your employer to ensure you receive appropriate care quickly and efficiently. If you’re unsure whether a service is covered through your employer, contact your HR department or speak with our Occupational Health team for guidance.

How can I schedule an appointment?

Appointments can be made by emailing [email protected] or calling the Occupational Health Call Center at 260-373-9300, option 1.

If you are an employer attempting to schedule an appointment for an employee, you can complete the Appointment or Results Request Form and then email to [email protected]. Once the form is submitted, our team will contact you to confirm appointment date, time and location. This form is NOT an authorization for services. Please send a Parkview custom authorization with your employee to their appointment.

Your employees are welcome to call 260-373-9300, option 1 to schedule their own appointment. We suggest they have their custom authorization in hand so they can refer to it and answer questions. To inquire if an appointment is required, contact our Employer Solutions team by reaching out to your Business Development Consultant, emailing [email protected], or calling 260-373-9017.

How long does it take drug screen results to process?

Negative rapid drug screen results are available shortly after the appointment.

Results for negative non-rapid drug screens generally take 48 hours to process. Results for positive non-rapid and rapid drug screens that are sent for further testing generally take 5-7 business days to process results.
