Community Health Needs Assessment

About

Every three years Parkview Health conducts a community health needs assessment (CHNA) to identify health needs for each community served by a Parkview hospital. Parkview uses the findings to identify and develop efforts to improve the health and quality of life for residents in these communities. Starting in 2022, the Health Services and Informatics Research (HSIR) group at Parkview’s Mirro Center for Research and Innovation designed and conducted the CHNA data collection and analysis activities. Prior to this, the assessment was performed by an outside organization.

 

Obtaining input from both individuals who reside in and those who provide health and community services in the communities is a required element of a CHNA. Publicly available population health data is also to be collected and included in the report. Surveys are used to gather input from individual community members and healthcare and social service providers (i.e., physicians, nurses, social workers) in order to understand local health concerns, needs, and service availability. An individualized CHNA is then developed for each county in which a Parkview Hospital is located.

 

Once the CHNA data is gathered and analyzed, it is shared with Parkview Health administrators, community organizations and community leaders to obtain their feedback and input. Finally, each Parkview entity for which a CHNA was conducted develops an implementation plan in which they outline their strategy, goals, objectives and activities to address the specific health needs of their community identified by their CHNA.
