What does Signature Care do with regard to my insurance?

Signature Care is a statewide network of healthcare providers located in Indiana and northwest Ohio that can be accessed through an employer's group health plan. Signature Care is a Preferred Provider Organization (PPO). When an employer chooses Signature Care as its PPO network, health plan participants receive medical services at a negotiated rate from contracted providers.

Your healthcare providers send your claims to Signature Care. We apply the negotiated discount and forward them to your Third Party Administrator, often thought to be an insurance company. The Third Party Administrator or TPA, will review and process those claims. You can find contact information for your TPA on the back of your ID card.

How do I find a participating provider? 

Use our convenient, online provider directory to find a participating (in network) provider. This directory is updated daily to be sure you have the most current information.

Is my physician in network?

To confirm that your provider is in network, try using our online provider directory. It is updated daily to be sure you have the most current information. You may also contact Customer Service at 1-800-666-4449.

Is the health facility that I am using in the Signature Care network?

Can I choose a provider who is not in the Signature Care directory?

The benefit plan at your employer may allow out of network benefits, however they are often far lower than when you stay in network. For your best benefit, use only those providers participating in the Signature Care network.
