Requesting your medical record might sound time-consuming or even confusing, but with Parkview MyChart, it’s quick, simple and right at your fingertips — no travel or phone call needed!
Having secure, direct access to your medical record helps you stay informed and involved in your care. Whether you’re tracking your health history, getting ready for an appointment or sharing your record before a new visit, MyChart makes it easy.
Requesting your record in MyChart
You can request your medical record anytime, right from the mobile app or your computer. Here’s how:

Log in to MyChart.

Go to the Menu and search or select Sharing Hub.

At the bottom left, choose Request formal copy of health record.

Answer a few quick questions and click Send request — and you’re done!
Once your request is received, our Health Information Management team will process it and let you know when it’s ready. Some requests can be processed automatically however, please know others can take up to 30 days to process if additional follow-up is necessary.
When your record is ready, you’ll get a message in MyChart and can download, save, forward and/or print it from the Document Center.
