Appointments & Referrals

Physician referral may be required by insurance

Some insurance companies require a referral from a family practice physician before seeing a plastic surgery specialist. Every insurance company is different. Before scheduling an appointment, call your insurance company and ask if your appointment and procedures will be covered. Co-pay will be due upon arrival.

No referral is needed for a cosmetic surgery consultation.

Schedule a private consultation

When considering a plastic surgery procedure, you’re encouraged to visit us for a personal consultation.

Your consultation allows you to get to know your surgeon and ask questions about your procedure. We want you to be an active participant in the process so it can be as educational and enjoyable as possible.

Call 260-425-6960 for more information or to schedule a private consultation.

Preparing for your appointment

When you come to your consultation, please bring the following with you:

  • Insurance card
  • Driver’s license or other state/federally issued identification
  • Current list of medications and allergies
  • Any paper work that you received
  • Current implant information (for patients who have breast implants)

A $75 consultation fee will be collected when you arrive for your cosmetic consultation and will be applied toward your procedure if you choose to schedule surgery.

What to expect during your consultation

An integral part of the decision-making process, a cosmetic consultation is the perfect time to talk with your provider about your options and any concerns you may have. It’s often a good idea to bring a friend, spouse or family member with you. They should be someone you are comfortable discussing your visit with and/or the person who will be helping you after your surgical procedure.

Medical Services

After you have met with your provider for your reconstructive consultation, we have a team that will call your insurance to obtain prior authorization for your surgery. Our schedulers will then call you with a surgery date, time and location. It is recommended that you call your insurance to verify authorization.

Cosmetic Services

After your cosmetic consultation and meeting with the provider, you will sit down with a patient care coordinator to review your free, detailed quote. You may choose to schedule your surgery at this time or at a later date.

When you are ready to proceed, the patient care coordinator will schedule a preoperative/physical appointment for you and your provider to review surgery details.

Payment is not required to schedule surgery. However, the amount must be paid in full within 10 days prior to surgery. Some patients elect to pay for these services using cash, credit card or bank check.
