A seamless referral experience

When you refer patients for orthopedic care, we want to make the process as smooth as possible—and it all starts with upfront communication. Our care navigation team is the bridge between referring physicians and our orthopedics providers. Navigators are available to answer questions, address concerns, and keep you up-to-date on our services, subspecialties, and more.

How to make a referral

For Parkview Physicians Group and Parkview Connect providers

Referring patients to a PPG – Orthopedics office located in Indiana

  • In Epic, use REF62—Ambulatory Referral to Orthopedics.
  • Class: Leave this selection as “Internal Referral.”
  • Priority: If the patient has an acute injury or fracture and needs to be scheduled into an urgent slot, change the priority to “Urgent.” Otherwise, leave as “Routine.”
  • Where are you referring to? Select “PPG Indiana.”
  • Reason for referral: Select the appropriate option
    • General Ortho (any orthopedic complaint that needs to be worked up by a specialist)
    • Surgical Consult (the patient has had a complete orthopedic workup and is ready to see a surgeon); this will be used most by OrthoExpress offices
    • Sports Medicine (athlete injuries and complaints)
    • Spine (ortho spine specialty)
  • Preferred Location: Select the appropriate option. A co-worker from the care navigation team or the selected office will reach out to the patient to schedule. If you are unsure which location is preferred or do not have a preference, leave this field unselected.
  • Comments: Add any scheduling/preference details here.

Referring patients to a PPG – Orthopedics office located in Ohio

  • In Epic, use REF62—Ambulatory Referral to Orthopedics.
  • Class: Leave this selection as “Internal Referral.”
  • Priority: If the patient has an acute injury or fracture and needs to be scheduled into an urgent slot, change the priority to “Urgent.” Otherwise, leave as “Routine.”
  • Where are you referring to? Select “PPG Ohio.”
  • Comments: Add any scheduling/preference details here.

Referring patients to a Parkview Sports Medicine (PSM) office

  • In Epic, use REF62—Ambulatory Referral to Orthopedics.
  • Class: Leave this selection as “Internal Referral.”
  • Priority: If the patient has an acute injury or fracture and needs to be scheduled into an urgent slot, change the priority to “Urgent.” Otherwise, leave as “Routine.”
  • Where are you referring to? Select “PPG Indiana.”
  • Reason for referral: Select “Sports Medicine.”
  • Is this related to a concussion? Select “Yes” or “No” as appropriate. If you select “Yes,” additional information will appear regarding patient eligibility for the PSM Concussion Clinic.
  • Preferred Location: Select the appropriate option. A co-worker from the care navigation team or the selected office will reach out to the patient to schedule. If you are unsure which location is preferred or do not have a preference, leave this field unselected.
  • Comments: Add any scheduling/preference details here.

Referring patients to an external entity (including an Ortho Northeast office)

  • In Epic, use REF62—Ambulatory Referral to Orthopedics.
  • Class: Select “External Referral.”
  • Provider Specialty: Select the appropriate specialty.
  • To Location/POS: Select the facility name to electronically send the referral.
  • Reason: Select the reason for sending the patient to an external resource.
  • Where are you referring to? Select “Other External.”
  • Location: Add text describing the referral location. This field is required, as it helps ensure that referrals are routed appropriately if an established electronic avenue does not exist for the referral location.
  • Comments: Add any scheduling/preference details here.
