The clinic care team model

Parkview’s team-based approach to patient care is unique. Parkview Neurosciences remains dedicated to developing outpatient clinics that meet the needs of our community and the physicians that refer individuals to us for evaluation and treatment. In conjunction with Parkview Physicians Group – Neurology and Parkview Physicians Group – Neurosurgery, the clinic care teams provide:

• A collaborative approach, which means that providers share their expertise when assessing your needs and collaborate when planning for your care. For your convenience, the clinic care team approach also means that you will be scheduled to see several providers in one visit, whenever possible.
• Customized treatment plan that identifies any health concerns you may have, as well as recommended actions or follow-up care by your primary care physician or the clinic team.
• Improved overall health and daily function with support from Parkview or another provider in the community, as needed
• Improved safety and independence because of our broad approach to understanding your health concerns and related factors such as the home environment.
