Hospital at Home is a service provided by a medical team dedicated to delivering compassionate, quality care in the comfort of your home. The expert team of doctors and staff will collaborate closely with you and your family to help ensure you achieve optimal healing.

What to expect

Your highly-coordinated care will include daily in-person or virtual visits from:

  • A physician
  • A nurse practitioner
  • A registered nurse and/or a community paramedic

Additional medical professionals will be available to you in person or virtually based on the personal care plan developed by your healthcare team. These medical professionals may include:

  • Consulting physicians
  • Patient care technicians
  • Pharmacists
  • Dietitians
  • Physical, occupational and speech therapists
  • Case managers
  • Other health professionals
Program requirements

To participate in the program you must:

  • Allow both in-person and virtual care team visits around the clock as deemed appropriate based on your medical condition.
  • Agree to use the electronic tablet provided for virtual visits.
  • Have access to a phone.
  • Ensure a safe environment for members of the care team who visit your home. This may include removing pets from the location in the home where care is being provided.
  • Be willing to stay in your residence for the duration of your care.
  • Refrain from smoking while members of the care team are in your home.
Preparing your home

The team will deliver needed supplies to your home and set up any electronic or medical equipment you may require, including:

  • A tablet to communicate with providers
  • Devices to monitor your vital signs around the clock
  • An IV pump to administer fluids and medications
  • X-ray and ultrasound machines, etc.

When you are well enough to be discharged from Hospital at Home, your care team can arrange for you to receive less intensive home health care services.

Cost of service

Parkview will coordinate the billing for all Hospital at Home services with your insurance provider. You will be responsible for any deductibles, co-payments or other out-of-pocket expenses based on your plan. If you have billing questions, please call 260-266-6700.
