Ambulatory care
Parkview offers a 12-month American Society of Health-System Pharmacists-accredited PGY2 ambulatory care residency for two residency candidates. Parkview’s PGY2 ambulatory care residency is designed to develop the resident into an independent, competent, confident and compassionate practitioner with the ability to provide excellent patient care in the ambulatory setting. This resident will develop a core skill set in chronic disease state management, patient interaction, communication, practice management, team building and time management. The program will assist residents in developing their precepting skills and will integrate precepting into various learning experiences.
Purpose Statement
The purpose of the Parkview Health PGY2 pharmacy residency programs is to build on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available.

Parkview Physicians Group
Parkview Physicians Group (PPG) includes over 900 providers in more than 40 specialties in more than 100 locations throughout northeast Indiana and northwest Ohio. Currently, Parkview pharmacists practice in a number of specialties including primary care, with a focus on diabetes, and participate in several multidisciplinary clinics through population health.
Anticoagulation Therapy Unit
Parkview’s Anticoagulation Therapy Unit (ATU) has provided anticoagulation management to the northeast Indiana region for over 20 years. Established in 1995, the various clinics serve over 3300 patients annually for warfarin education and management. The ATU also has a patient self-testing program, where patients can receive a meter and test strips through Parkview’s ATU with warfarin management by our highly-trained pharmacists.
Parkview Heart Institute
As the region’s only dedicated heart hospital, Parkview Heart Institute is focused on prevention, early detection and treatment of heart disease. Specialized cardiac ICU, acute care and accredited congestive heart failure and cardiac rehabilitation centers are all together in one location. Outpatient specialty clinics support patients with vascular, valve, hypertension, lipid and genetics-related heart conditions. Pharmacists are integrated in both the inpatient and ambulatory settings, with a focus on heart failure, hypertension, and lipids in the ambulatory clinics

Parkview Health pharmacy residents are required to complete twelve months of practice experience during their residency. Additional rotation opportunities may be arranged at the residents’ request.

Application and information
Residency candidates must have a Doctor of Pharmacy degree and be eligible for pharmacist licensure in Indiana. Candidates must have successfully completed an ASHP accredited PGY-1 Residency. Candidates must also register for the Matching Program and have a National Matching Services (NMS) applicant number.
