Residency candidates must be graduates of a College of Pharmacy program accredited in the U.S. and must be eligible for licensure in the State of Indiana.  Candidates must also register for the Matching Program and have a National Matching Services (NMS) applicant number.

According to guidelines by the Pharmacy Online Residency Centralized Application Service (PhORCAS), candidates must submit the following information no later than
January 15:

  1. Curriculum Vitae
  2. Letter of Intent
  3. Three letters of recommendation via PhORCAS Standard Electronic Reference Form
  4. Transcript from College of Pharmacy
