Making your advance care plan a priority

Choosing to make your future healthcare wishes known is a powerful act. National Healthcare Decisions Day on April 16 reminded us that planning for future medical care is important. But these conversations shouldn’t be limited to just one day—there’s always time to start making decisions that reflect your personal values and goals.

Healthcare choices should match what matters most to you! Treatments work better when they support your wishes. Clear communication helps your loved ones and care teams give the right care for you. At Parkview Health, a free process called advance care planning (ACP) helps ensure these preferences are documented and honored. Taking the time to plan today is meaningful while bringing peace of mind for the future.

What is advance care planning?

ACP is the process of coming to understand, reflect on, discuss, and plan for a time when you cannot make your own medical decisions. Through this process, you can create a plan called an advance directive. ACP will help you:

   

Choose one or more healthcare agents to make medical decisions for you if you can’t make them for yourself.
 

   

Prepare and make a priority for important talks with your care team, healthcare agent and others.
 

   

Document what matters most to you and how that may affect the type of care you choose.
 

   

Share your goals, values and wishes for care with others.
 

   

Review and update your advance care plan and advance directive periodically and as needed.
