Fall Prevention

Falling is the No. 1 cause of trauma-related injuries for people ages 65 and older. Parkview’s Fall Prevention Clinic can help you or someone you love reduce the risk of falling.

At the Fall Prevention Clinic, patients experience:

  • Easy access to specialists in neurology, neuropsychology, physical therapy, occupational therapy, pharmacy, and social work.
  • Convenience, as you are assessed by only the appropriate team members, typically in one visit.
  • Collaborative care management, as a multi-disciplinary team reviews your assessments and develops a treatment plan and recommendations specifically for you.

A physician referral is needed for someone to be assessed at the clinic. A referral to the Fall Prevention Clinic can be made by an individual’s primary care physician or the Parkview Welcome Clinic team. 

This Fall Prevention Clinic is located on the second floor of Parkview Hospital Randallia, within the Parkview Center on Aging & Health.

Medication Assistance

The Parkview Medication Assistance Program (MAP) connects the patient, healthcare provider and pharmaceutical manufacturers for the purpose of providing prescription medications to uninsured or underinsured patients.

How does the program work?

Patients in need of assistance with prescription medication costs can apply to Parkview MAP which utilizes Pharmaceutical Assistance Programs, product replacement and co-pay assistance programs to provide medications to uninsured or underinsured patients.

Who is eligible for the program?

  • Patients unable to pay for prescription medications
  • Patients with full, limited or no prescription insurance coverage.

Note: Patients with insurance coverage through Medicaid are not eligible for this program.

Application process

It may take up to 4 – 6 weeks to process your application and begin medication assistance. During that time, you are responsible for filling/refilling your own medications. Applicants may be required to provide the following*:

  • Application** (to be completed by the MAP team, healthcare provider and patient)
  • Proof of health and/or prescription insurance (if applicable)
  • Proof of residency
  • Verification of income

 * Pharmaceutical manufacturers may ask you to meet additional requirements and/or supply additional documentation.

** Application will cover all eligible medications on your medication list

LifeBridge Senior Program

If you’re 65 or older, Parkview’s LifeBridge Senior Program can assist you with navigating any number of life’s challenges.

  • Do you sometimes feel alone?
  • Have you lost interest in activities you once enjoyed?
  • Do you ask yourself, “How can I get over this slump?” but the answers just don’t come?

 We are dedicated to helping you enjoy life again. The team at LifeBridge provide compassionate care to help you strengthen your confidence and ability to cope with life’s more challenging issues. Through a combination of one-on-one meetings, group discussions and engaging activities, you can rediscover your independence and a desire to live life to the fullest.

Most participants visit this outpatient program two days per week for six to 12 weeks. The multidisciplinary team will also provide follow-up care once you complete the program. LifeBridge is currently offered in two locations:

Parkview Health & Fitness Center

The Parkview Health & Fitness Center offers a fully equipped workout facility with a diverse range of fitness classes and access to several fitness assessment tests. Our professionally trained staff and certified instructors will work with you to develop personalized programs that meet your needs. 

Located at 3000 E. State Blvd. in Fort Wayne, the Health & Fitness Center offers:

  • More than 100 group exercise classes each month
  • Supervised exercise programs, in which our staff members monitor blood pressure and heart rate before, during and after each session
  • Personal trainers for one-on-one supervision and development of a plan to accomplish your personal fitness goals
