Whether you’re recuperating from an illness or taking care of a loved one, there is no place like home to make you feel comfortable. Just being surrounded by family, friends and familiar things feels better than being in a hospital.

Parkview Home Health & Hospice provides professional health care in the privacy of your home and many area long-term care facilities. As a service of Parkview Health, we are a not-for-profit agency providing services to meet your special needs. We are also licensed by the state of Indiana, accredited by the Joint Commission on Accreditation of Healthcare Organizations, and certified by Medicare and Medicaid, with locations in Columbia City, Fort Wayne, Huntington and Kendallville, serving northeastern Indiana.

Working in cooperation with your personal physician, our healthcare professionals provide a full range of homecare services, including:

  • Home I.V. education and services such as total parenteral nutrition, antiobotics, chemotherapy, enteral therapy, hydration, respiratory therapy and pain management
  • Hospice care for the physical, emotional and spiritual needs of the patient and family during a terminal illness Read more about hospice care
  • Home health equipment such as oxygen and supply coordination and education, wheelchairs, nebulizers, CPAP/BiPap, agnea monitors, hospital beds and ambulatory aids
  • Physical, occupational and speech therapists to teach and assist with daily activities
  • Medical social workers to provide you and your family with emotional and social support as you recover from your illness or injury
  • Skilled nurses to care for your health needs and teach you self care techniques
  • Psychiatric homecare to provide a smooth transition from a psychiatric hospital to home
  • Home health aides to assist with bathing, meal preparation and other important daily living needs
  • Registered dietitians to develop your specific nutritional program
  • Palliative care and respite is also available

We are available 24 hours a day, seven days a week by calling 800-363-9977.  Our office hours are 8 a.m. to 5 p.m., Monday through Friday. After office hours or if an emergency arises, a registered nurse is on call to assist patients and their families.
