Brain Injury Program

The Brain Injury Program at Parkview Rehabilitation Center was specifically created to treat patients of all ages who have suffered a brain injury due to trauma or illness. The program at Parkview meets rigorous national standards for quality and is focused on delivering the most favorable results to patients.

Each patient and his or her family members work as partners with the rehab team to create an individualized team care plan following an evaluation of the patient’s condition.

Certified brain injury specialists
To provide the highest-quality rehabilitation care possible, our rehab team includes 15 staff members who have received specialized certification from the Academy of Certified Brain Injury Specialists. These Parkview rehabilitation professionals have a detailed understanding of the impairments in physical function and behavior caused by specific types of brain injury. Well-versed in effective treatment approaches, these members of our rehab team assess available therapies and assistive equipment on an ongoing basis, incorporating new choices into patients’ treatment plans as appropriate.

Rancho Los Amigos Cognitive Scale
Patients admitted to the Brain Injury Program must have at least a generalized response to external stimulation, which is a Level II on the Rancho Los Amigos Cognitive Scale. This neurological tool is widely used by rehabilitation facilities to describe and measure a patient's level of functioning and progress over an extended period of time. The Rancho scores also guide clinicians in the development of individualized treatment programs.

Some third-party payors – insurance carriers – require that patients be able to demonstrate behaviors at Level IV or higher.

Rancho Los Amigos Cognitive Scale

Level I

No Response

Unresponsive to all stimuli

Level II

Generalized

Inconsistent, non-purposeful, non-specific reaction to stimuli. Responds to pain, but response may be delayed.

Level III

Localized

Inconsistent reaction directly related to type of stimulus presented. Responds to some commands. May respond to discomfort.

Level IV

Confused

Disoriented and unaware of present. Agitated events with frequent bizarre and inappropriate response behavior. Attention span is short, and ability to process information is impaired.

Level V

Confused
Inappropriate
Non-agitated

Non-purposeful random or fragmented responses when task complexity exceeds abilities. Patient appears alert and responds to simple commands. Performs previously learned tasks, but is unable to learn new ones.

Level VI

Confused Appropriate

Behavior is goal-directed. Responses are appropriate to the situation with incorrect responses due to memory difficulties.

Level VII

Automatic, Appropriate

Correct routine responses, which are robot-like. Appears oriented to setting, but insight, judgment and problem solving are poor.

Level VIII

Purposeful and Appropriate (Stand-by assist)

Consistently oriented to person, place and time. Able to recall and integrate past and recent events. Depressed, irritable, low frustration tolerance, easily angered and argumentative.

Level IX

Purposeful and Appropriate (May request stand-by assist)

Independently shifts back and forth between tasks and completes them accurately for at least two consecutive hours. May be easily agitated and depressed. Able to monitor appropriateness.

Level X

Purposeful and Appropriate (Modified Independence)

Able to handle multiple tasks simultaneously in all environments, but may require periodic breaks. Irritability and low frustration tolerance when sick, fatigued or under stress.
