Since we opened our doors at State Boulevard and Randallia Drive in Fort Wayne back in 1936, Parkview has been committed to providing area residents with quality services that improve their personal health. Now nearly 60 years later and following expansion throughout northeast Indiana, our priority remains meeting the needs of today’s throughout the region.
To help us fulfill this commitment, Parkview is working with the Fort Wayne – Allen County Department of Health and Indiana University Purdue University – Fort Wayne (IPFW) to conduct a community health needs survey in the next few weeks. IPFW will randomly select individuals within certain zip codes in northeast Indiana communities – including Allen, Huntington, LaGrange, Noble and Whitley counties. If you are selected, a brief survey will be mailed to your home. Questions will ask about health status, behaviors that affect your health and additional services you think are needed.
When you complete the survey, you will help to identify the community’s health needs today and tomorrow. This unique information will help us plan how best to meet the critical healthcare needs of our community.
