Health Partners

Parkview collaborates with other community organizations to address unmet health needs through community health initiatives. Guidelines can be found in the partner application for community organizations who wish to enter into a collaborative relationship.

If interested in becoming a health partner, complete the Health Partner Application.  Organizations serving LaGrange County residents should submit a LaGrange Health Partner Application.

Current health partners are required to submit a Partner Progress Report on a regular basis. 

Note: a logical model form is required to be included with the partner application.

Honoring Our Health Partners
Community Health Improvement (CHI) Week is celebrated nationally each year to increase understanding of such activities and the individuals who benefit from them.
 
In recognition of CHI week 2012, several health partners are featured here. Through such partnerships, Community Health Improvement helps provide individuals with access to preventive health services, resources and education that helps to maintain and improve their health.
 
Parkview’s mission is to improve the health of individuals in all communities we serve. Our Community Health Improvement Program is one way in which mission translates into traditional patient care as well as health services beyond our facility walls. Under the CHI Program, each hospital in the Parkview Health family returns 10 percent of its annual operating surplus to various community-focused health initiatives.
