Medication Assistance Program

Parkview LaGrange Hospital has announced a new Medication Assistance Program (MAP). The program provides assistance for patients who live in, or whose physician practices in LaGrange County and who have limited or no prescription insurance. MAP addresses two types of medication needs: short term through an Emergency Voucher Program and long term through the Maintenance Medication Program.

MAP’s Emergency Voucher Program is funded by generous donations to the Parkview LaGrange Foundation. Through this program, patients who need medication for a short-term illness, such as an infection or virus, or assistance with their initial prescription for maintenance medications, may be issued a medication voucher from the Parkview LaGrange Foundation that can be used to have their prescription filled at a designated local pharmacy.

The Maintenance Medication Program serves as a link between the patient, his or her physician and the large pharmaceutical companies. This program provides long-term medication assistance to LaGrange County residents or eligible patients of LaGrange County physicians.

In order to be eligible for assistance through MAP, the patient must meet several criteria, among which are:

  • Be referred to the program by their physician.
  • Provide proof of residency in LaGrange County or are a patient of a LaGrange County physician.
  • Be at least 18 years old (parents or legal guardians may apply on behalf of their dependents).
  • Have limited or no prescription insurance or be experiencing a financial emergency.
  • Additional criteria may apply depending upon the program.

All patients must set up an appointment with the medication assistance coordinator to apply for the MAP programs. Parkview LaGrange Hospital is a not-for-profit community hospital and is licensed as a 25-bed critical access hospital to provide acute care services to the people of LaGrange County and surrounding communities.  In July of 2008 Parkview LaGrange opened its new, $25 million hospital facility.
